When to Arrive:

For your first appointment, you will want to arrive 15 minutes prior to your scheduled time to complete the necessary forms or you can find the forms by clicking here to fill out prior to your arrival. For subsequent appointments, please be sure to arrive at your scheduled time. Prompt arrivals assure adequate time to evaluate and to advise you of your progress.

Cancellation Policy:

Please note there is an overwhelming demand for our services. If you miss an appointment, we cannot help another patient in your place. We require a minimum cancellation notice of 24 hours (please call Friday to cancel Monday appointments). Any missed appointment or any cancellation notice of less than 24 hours prior to your appointment, is subject to a fee of $100.00.

Payment Policy:
